DXM
In brief
A cough suppressant that turns dissociative at high doses (NMDA antagonist), in successive plateaus. Serotonergic risk, and danger from multi-ingredient syrups.
Tolerance
Tolerance sets in quickly with repeated use, pushing escalation. It inhibits CYP2D6 (so its own metabolism and that of other drugs); chronic use is linked to cognitive and, depending on the syrup, hepatic risks.
Contraindicated combinations
MAOIs: a dangerous combination (hyperthermia, serotonin syndrome). Avoid other serotonergics (SSRIs/SNRIs, MDMA, tramadol) and depressants (alcohol, opioids). Beware multi-ingredient syrups (paracetamol, antihistamines).
